RN Care Mgrs to conduct telephone Risk Assessment for Care Plans.
Full-time temporary position for approximately 3-4 months.
Our Client, La Rabida Hospital, located at Lake Michigan on the south side of Chicago has a need for 3 license RNs to establish Care Plans for medically fragile children who are being enrolled in Medicaid MCO Programs. The goal to complete Care Plans 3 to 4 per day.
Purpose of positions
RNs will conduct thorough health, socio-economic, behavioral, self-management and risk status assessments telephonically with members, parents & providers. This care plan established will be based on documented needs. You will conduct proactive and outreach calls with parents and physicians involved with the enrollee’s plan of care. You must work collaboratively with clinical teams and families in developing health and lifestyle goals so as to improve overall patient health outcomes. This is a full time 8am-5pm position, but candidates need to be flexible to work 11am-7pm or occasional Saturdays as needed reach parents on the phone.
A BSN and a current IL license is required. Experience or a certification in peds nursing, case management or other specialties is desired. Experience completing health risk assessment surveys required, including experience interviewing families/patients and documenting health, behavioral and social histories. Experience working with at-risk populations also required.
Pediatric experience & case management experience in a safety net environment is strongly preferred. Experience with managed care organization or population management preferred.
Bi-lingual capacity a strong plus. Excellent written and verbal communication skills, excellent interpersonal skills, demonstrated ability to manage client/provider relationships. demonstrated knowledge and expertise in pediatrics, any related specialty area, and family education.
